  • Reason for cancelling

  • Contact phone number

  • Email Address

Cancellation Fee:

Members within 12 months from the starting date of a 12-month agreement will be charged $100.00


Accounts can not be cancelled if they have unpaid charges

Email cancellations are offered as an alternative option for the required mail-in cancellations outlined in membership agreements.


Accounts will only be closed after members receive an email response.
